Who May Need a Covid Booster Shot ?

                                                                                                                                It's important to know that there is a                                                                          difference between an additional dose of the                                                                covid vaccine and a booster dose. It's a bit                                                                    confusing, but they are two different things.                                                                       The  recommendations to administer an                                                                  additional dose of Pfizer-BioNTech COVID-19                                                              vaccine or Moderna COVID-19 vaccine are      limited to individuals who already received two doses of one of these vaccines and  are moderately to severely immunocompromised. The guidelines state that moderately to severely immunocompromised individuals are those who have:

  • Been receiving active cancer treatment for tumors or cancers of the blood

  • Received an organ transplant and are taking medicine to suppress the immune system

  • Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system

  • Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)

  • Advanced or untreated HIV infection

  • Active treatment with high-dose corticosteroids or other drugs that may suppress the immune response

 

Additional considerations:

  1. Janssen/ J&J vaccines are not approved for additional doses in immunocompromised patients.

  2. Immunocompromised patients who were previously vaccinated with Janssen/ J&J vaccine are not eligible for an additional dose at this time.

  3. In eligible immunocompromised patients who have received 2 previous doses of an mRNA vaccine (Pfizer or Moderna) the additional dose should be spaced at least 28 days after the second mRNA dose.

  4. The manufacturer of the additional mRNA dose should match the manufacturer of the mRNA vaccines given as dose 1 and dose 2 when possible. If the matching product is not available, it is acceptable to use the alternate mRNA COVID-19 vaccine. Do not use Janssen/ J&J vaccine.

  5. Additional doses are not recommended for the general population with normal immune function.

  6. Physicians and other prescribers may not write prescriptions for any vaccine that is provided under an Emergency Use Authorization (EUA) for any unlabeled or unapproved use.
